Octagonal foot, short cylindrical stem; body in the form of an octahedral cup, expanding upward. From the front, in the frame with truncated corners - an inscription stylized as a kraut: "Pamiątka / z / Szczawnica / J. K.". On the back a view of the building with a signature topped with a cross and the caption: "Chapel". Also, stylized acanthus-volute decorative motifs.
Western condition: minor chip on the base.
